Emergency dental care in the EU: EHIC, payment and reimbursement
What an EHIC can cover during a temporary EU stay and which records to keep if you pay for dental care.
Created Updated

The EHIC follows the host country’s rules
A valid European Health Insurance Card (EHIC) gives access to medically necessary public-system care during a temporary stay in another EU country on the same terms as locally insured people. This can include holidays, work trips or study. It is not limited to life-threatening emergencies, but does not cover treatment that was the purpose of the trip.
The treating professional assesses what care is necessary and whether it can wait until your return, considering your condition and length of stay. Dental coverage also depends on the host country's public benefits. Bulgaria's annual dental package does not automatically transfer to a foreign practice.
The EHIC does not guarantee free treatment. Local contributions and limited dental benefits may apply. It does not cover private care outside the applicable public system or transport home to Bulgaria. For a life-threatening emergency, call 112, free throughout the EU.
What to establish with the practice
Explain that you are temporarily visiting, insured in Bulgaria and have an EHIC. Ask whether the practice provides the required service through the public system and accepts the card for it. An emergency-clinic sign does not establish this.
Present the card and ID. As far as your condition allows, ask what treatment is proposed, what you must pay and whether the amount is a local contribution or a fully private price. Clarify any services charged under different arrangements.
If you do not have the card, ask NHIF about a provisional replacement certificate. Verbal assurance that you are insured may not suffice. Necessary care may require payment followed by an application for reimbursement assessment.
Consult the official country-by-country EHIC information for providers and possible charges.
Why Spain and the Netherlands differ
In Spain, dentistry is generally outside public coverage, with limited exceptions including certain acute conditions and some children's services. Sudden pain does not mean every necessary treatment at a chosen private dentist will be paid through the EHIC.
In the Netherlands, public dental coverage mainly concerns children. Adults generally pay and cannot obtain reimbursement under that local arrangement. Possible reimbursement from the country of insurance is assessed separately and does not automatically repay the whole invoice.
These examples were checked in October 2026. Your procedure, provider and current local rules determine the position. NHIF's 2026 travel replies likewise refer to clinical assessment and host-country conditions.
Keep records of the treatment you paid for
Keep an itemised invoice, proof of payment and clinical documents showing the patient, dates, practice, procedures and amounts. Request copies while still there and retain the practice's contact details.
NHIF has separate conditions and applications for Regulation (EC) No. 987/2009 and cross-border care under Directive 2011/24/EU. Describe the country, reason for travel, treatment, provider and EHIC use to NHIF so the appropriate route can be established; do not choose solely from a form's title.
EHIC-related reimbursement usually follows the treatment country's rules and tariffs. If the card was not used, including at a private provider, another route may apply, checking coverage and payment limits in the country of insurance. The European Commission explains the distinction; NHIF assesses your claim. Neither route promises full reimbursement of a private price.
Before applying, check the current form, required attachments and any need for originals or translations. Keeping documents prepares your claim; it does not guarantee approval or full payment.
For questions about EU emergency-care expenses, NHIF gives fin_assist@nhif.bg in its reply of 11 May 2026. Describe the case first and confirm how documents should be supplied. An email enquiry is not a reimbursement application.
Travelling specifically for treatment
If planned dental treatment is the purpose of your trip, the EHIC is not its funding document. Check planned-care conditions and any prior authorisation with NHIF before committing to costs. A similar name to a Bulgarian covered procedure does not establish payment entitlement.
On return, organise your records for the next dentist. Our dental documents guide explains useful information for a handover.
