Allergy assessment before dental care: referrals and payment

When an anaesthetic reaction needs assessment and how NHIF referrals and skin-testing coverage work in Bulgaria.

Clarify the reaction before choosing a test

Tell the dentist what happened with a previous anaesthetic: the symptoms, when they started and any treatment received. This helps determine whether an allergist consultation is needed and how to plan dental care.

Faintness, palpitations or trembling after an injection do not alone prove allergy. NHS Specialist Pharmacy Service explains that true allergy to the local anaesthetics used is rare; anxiety and other drug effects can also cause reactions. A clinician must establish the cause. Do not add or remove an allergy diagnosis from an online description alone.

Breathing difficulty or swelling of the tongue or throat after a medicine needs immediate medical help. Alert the team immediately if you are in the surgery; otherwise call 112. Planned testing is not a response to an evolving severe reaction.

The route to an NHIF allergist

In its 17 September 2026 reply, concerning a child before dental treatment, NHIF describes this route:

  1. When necessary, the dentist refers to the GP using consultation form MZ 119A.
  2. The GP examines the patient and may issue referral No. 3 to a clinical allergist, depending on the condition.
  3. The allergist examines the patient and decides which investigations are needed. NHIF-funded skin allergy testing requires specialist referral No. 3A.

These forms serve different purposes. A dental note requesting an allergy test does not automatically replace the referral for the specific highly specialised activity. Consultation and testing may take place at different facilities.

When booking, explain the previous reaction or the question to be resolved before dental care. Confirm that the allergist works with NHIF and where the prescribed investigation can be performed.

When NHIF pays for skin testing

NHIF pays for skin allergy testing, code Z01.5, when prescribed by a specialist on referral No. 3A and performed at a facility contracted for that exact activity. An NHIF contract for other consultations is insufficient.

Confirm the prescribed test, acceptance of the referral and any patient charge with its basis. A private allergy-testing package does not establish what NHIF covers. Different tests and consultations have different conditions.

In its 4 February 2026 reply, NHIF also lists an oral drug provocation test within the allergist's package. This does not make it necessary for every patient before anaesthesia. The specialist chooses the investigation and ensures it is performed safely.

For a proposed blood test or another investigation, check its exact name, prescribing requirements and coverage. The phrase “allergy test” alone does not identify who pays.

What to bring to the consultation

Bring records of the previous reaction and a medication list. Record the anaesthetic name if known; otherwise ask the practice that administered it. Note roughly how soon symptoms began and whether treatment or emergency help was needed. Do not fill gaps with guesses.

Ask the allergy clinic about preparation for your prescribed test. Do not stop medicines yourself based on generic allergy-test instructions; preparation depends on the test and your health.

Obtain the written conclusion and results, show them to the dentist and ask how they affect the treatment plan. Keep them with your dental and medical documents.

Planned dental treatment alone does not mean every patient needs allergy testing. History and examination determine need. For children, this referral route is separate from the annual dental package.
